    The West Yorkshire Keyworker Service (WYKWS) supports children and young people aged 0–25 with autism and/or a learning disability who have complex needs and are at risk of mental health inpatient admission. <br/>The service plays a key preventative and system‑coordination role across health, education, social care and the voluntary sector, operating across Bradford, Calderdale, Kirklees, Leeds and Wakefield.<br/><br/>The service will provide:<br/><br/>Dedicated Key Workers delivering:<br/>Assessment, engagement, care coordination, advocacy and ongoing case management<br/><br/>Multi-agency working across:<br/>NHS services, Local Authorities, education and VCSE partners<br/><br/>Support for:<br/>High-risk and complex cases, including safeguarding-sensitive situations <br/><br/>Participation in:<br/>Dynamic Support Registers and Care, Education and Treatment Reviews <br/><br/>The service aims to:<br/>Prevent hospital admissions and escalation<br/>Improve coordination of care across systems<br/>Enhance outcomes and experience for children, young people and families.
