School nurses identify vulnerable students, reduce health inequalities, provide support for chronic disease management, and contribute to safeguarding by working with families and other health teams to provide early intervention and support. Delivery of the service is divided into universal, targeted and specialist\nHealthy weight support - This service will provide support for families through key transition points, from milk to starting solids. This will include strengthening the pathway for families as they move out of health visiting and into wider health systems including into support for the healthy lifestyles pathways to raise awareness and strengthen referral pathways.\nThese services should be made available to improve outcomes for infants up to 5 years and their primary care givers. Delivery of the service is divided into universal, targeted and specialist.\nStart for Life services - Brent was one of 75 local authorities to receive funding to develop Family Hubs across the borough, including the development of an improvement in access to Perinatal mental health services and breastfeeding support. At present, the Brent has commissioned a Perinatal Mental Health Service with Central North West London NHS Trust to deliver a mild to moderate service for families as well as peer support for families delivered by Home Start."
