Published requirement descriptionWM5G Ltd is seeking to procure a configurable Digital Discharge Service (DDS) Workflow solution to support the coordination of hospital discharge-to-home pathways across NHS partner organisations. The solution will provide a single operational platform through which WM5G Ltd staff can manage referrals, patient workflows, consent, tasks, communications and service performance, replacing fragmented processes that currently rely on multiple supplier systems and manual administration.
The procurement covers the software platform, implementation, integration, hosting, training, support and maintenance required throughout the contract term. The solution will act as the operational orchestration layer for the wider DDS service, providing a unified view of patient journeys while integrating with existing Home Readiness, Remote Monitoring and Asset Management services. It is intended to improve the efficiency, scalability and governance of discharge services while maintaining clear accountability and visibility across multiple organisations and care settings.
The platform must support the complete discharge workflow, from patient identification and referral through triage, enrolment, discharge preparation, discharge, monitoring and completion. Core capability areas include referral and onward-care management, workflow and task management, consent management, reporting and analytics, administration and role-based access control, standards-based integration, and data quality management.

WM5G Ltd has defined a mandatory high-level architecture that suppliers must conform to. The solution must support API-first integration, HL7 FHIR interoperability standards, UK cloud hosting, strong identity management and secure integration with external systems. The architecture is designed to support rapid onboarding of new customers and providers while minimising operational complexity and vendor lock-in.
The authority's desired outcomes are improved patient flow, reduced discharge delays, better patient and carer experience, reduced administrative burden, improved operational oversight, higher data quality and a platform capable of scaling across multiple regions and thousands of patients. Compliance with NHS and regulatory standards, including DCB0129, DCB0160, DTAC, DSPT, Cyber Essentials Plus and UK GDPR, is mandatory.
Contract award will be based on the Most Advantageous Tender (MAT), assessing suppliers' ability to meet both the mandatory architectural requirements and the operational outcomes defined within the Requirements Compliance Matrix.

Any Pass/Fail, Mandatory or Critical questions as stated in PSQ - Conditions of Participation, Form T4: Tenderer's Proposed Solution, and Form T16 - Requirements Matrix The Authority's maximum budget for Year 1 of this Contract is £250K. This includes all mobilisation / implementation / integration and year 1 recurring costs as per Form T6 - Pricing Schedule. Any Tender with a total evaluated Year 1 price exceeding 120% of the Authority's maximum budget (£300K) may be deemed unaffordable and excluded from further evaluation. Should a Tenderer fail any of these stated requirements their tender submission will not have met the minimum requirement and may not be considered any further. During this stage no other elements of the tender submission will be considered or reviewed. If a Mandatory document has not been returned, we will send a web correspondence on one occasion for your immediate return to rectify an omission mistake. Stage 2: Full Tender Evaluation At this stage the complete tender submissions will be fully evaluated and each Tenderer will be marked in line with the process detailed in the sections below. At this stage bidders will be down-selected to a maximum of 3 suppliers to move onto the next stage. Tenderers will be notified and appropriately debriefed if they have moved on to the next stage or not. Stage 3: System Demonstrations and Dialogue At this stage the down-selected bidders will be required to demonstrate, but not limited to the following: system capability, functionality, ease of use, expected outcomes, integrations. They must also guide us through the actions and processes we can self-customise. Dialogue will take place to clarify functionality. This stage will not be scored and will be used to clarify and moderate scores where applicable. Stage 4: Final Clarifications This stage will provide a formal opportunity to clarify any outstanding questions WM5G Ltd may have in respect of any aspects of your bid submission. Stage 5: Preferred bidder nominated The Preferred bidder will be nominated based on the "Final Set Score" achieved in the above stages. (Quality Score and Price Score) Stage 6: Award At this phase of the tender any applicable 8-day mandatory standstill period will have been successfully completed and compliant tenderers notified and appropriately debriefed if they have been successful/ unsuccessful. Market Engagement Notice was not necessary as there is no legal requirement to publish such a notice in order to undertake pre-market engagement, and the Authority was satisfied that the engagement was proportionate, did not distort competition, and did not provide any supplier with an unfair advantage in the subsequent procurement process. Any information obtained through pre-market engagement has been used solely to inform the procurement strategy, specification, commercial approach and evaluation methodology. Any extension will be at the Authority's sole discretion and subject to satisfactory supplier performance, continued operational requirements, and the achievement of value for money.
