Extended hours from the PCAS Framework
Provide the extended hours of the GP out of hours 'Hubs' with GP, Paramedic and nurse appointments on weekday evenings and on weekends to support patients to access appropriate urgent care in Sutton.
Provide dedicated paediatric clinics within the hubs to support low acuity presentations to be seen in primary care, and support avoided Paediatric ED attendances.
To offer additional Hub clinics for both adults and paediatrics on weekends and weekday evenings in addition to the core contracted 'Extended Access' provision provided by SPCNs on behalf of Sutton GP Practices.
Reporting on agreed KPIs.
Provide support for the evaluation that will be required.
Provide operational support to flex the offer in line with demonstrated need.
To provide clinical leadership for the project and Board level governance to support the scheme from inception through to delivery and evaluation.
