Published requirement descriptionThe Specialist Assessment of the Frail Elderly (SAFE) services delivers proactive care for people at risk of, or living with, frailty in North Lincolnshire. The GP-led specialist service undertakes holistic Frailty Assessments for a proactively identified patient cohort and develops person-centred care and support plans, including advanced care planning where appropriate, for those individuals.
The service is delivered in the patient's own home or in a clinic setting, based on individual's need. The service provides both 'routine' and 'rapid access' appointments. Rapid access appointments are offered for those patients where there is a high risk of hospital admission within the next 2 weeks, or where a patient has been discharged from Same Day Emergency Care (SDEC) or acute Frailty service and is at risk of readmission.

This is a Provider Selection Regime (PSR) intention to award notice. The awarding of this contract is subject to the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award. The publication of this notice marks the start of the standstill period. Representations by providers must be made to the relevant authority by 23:59hrs 27 March 2024.
