Provision of Primary Care Medical Management and Support

Medical input, care management, support and advice for the residents in Lings Bar<br/>Rehabilitation centre, (72 beds). Ensuring the service is well led in-line with CQC domain definitions and fully supported to deliver high quality primary medical level of care and oversight for residents.<br/><br/>Patients may have transferred from acute care or require step up from community-based care, are medically fit and require a period of assessment, rehabilitation recovery and/or reablement to achieve discharge into an appropriate community setting.<br/><br/>This notice is to confirm the intention of Nottinghamshire Healthcare NHS Foundation Trust to award a contract under The Health Care Services (Provider Selection Regime) Regulations 2023 Competitive Process.<br/>This is an existing service being awarded to a new provider.<br/>The contract start date will be the 1st of January 2025 for a term of 12 months, and its expected value is £150,000.00.<br/><br/>Price is not the only award criterion and all criteria are stated only in the procurement documents.
