The target population includes residents from areas of socioeconomic deprivation, Black African and Black Caribbean communities, South Asian communities, Eastern European residents, people with severe mental illness or learning disabilities, and people with long-term conditions such as hypertension and diabetes.\nThe service will provide structured, evidence-based interventions including behavioural counselling, motivational interviewing, culturally tailored engagement, community outreach, accessible delivery locations, and digital options where appropriate. It will complement rather than duplicate existing Croydon Council universal healthy behaviour services, with clear referral and signposting arrangements between services.\nThe three main service components are:\nWeight management: targeted support for eligible adults aged 25-64 with BMI above the defined threshold and additional clinical or population risk factors, with outcomes focused on programme engagement, completion and clinically meaningful weight loss.\nSmoking cessation: targeted support for smokers aged 40-74 with diagnosed hypertension and/or diabetes who have not used stop smoking services in the previous 12 months, with outcomes focused on treatment starts and CO-validated quit rates at 4 and 12 weeks.\nAlcohol reduction: targeted support for North Croydon residents aged 40-64 with diagnosed hypertension and/or diabetes and increasing or higher-risk alcohol use, with outcomes focused on brief interventions, extended support, reduced AUDIT-C scores and signposting to specialist services where dependence is indicated.\nThe provider will be expected to establish referral pathways from GP practices, Croydon Council services, VCSE organisations and self-referrals, subject to validated eligibility evidence. The provider must also work closely with local partners, including primary care, Croydon Council, voluntary and community organisations, adult healthy behaviour services, smoking cessation services, weight management services, substance misuse providers and wider social support services.\nThe service will be delivered under a block contract for the period 1 April 2027 to 31 March 2030, with an option to extend by one year. The annual contract value is £184,539 (Inclusive of VAT) and the total contract value is £738,156 (Inclusive of VAT), with indicative component allocations for smoking cessation, alcohol reduction and weight management. The programme is designed to address key health inequalities identified through locality population health analysis and will focus on interventions that improve access to healthcare and address wider determinants of health.\nThe proposed service consists of three targeted prevention programmes aligned to the differing population needs of Central East and Central West Croydon:\n• Central East Croydon - Digital Inclusion and NHS App Uptake Programme\nSupporting residents aged 40 years and over who experience digital barriers to accessing health services and who are not currently registered with the NHS App. The programme will provide targeted outreach and personalised support to enable residents to access NHS digital services, improve self-management of long-term conditions and strengthen access to healthcare.\n• Central West Croydon - Welfare, Financial and Housing Advice Programme\nProviding structured casework support for adults living in IMD 1-3 areas who experience financial or housing insecurity and whose wellbeing is significantly affected by these pressures. The programme will prioritise individuals with moderate-to-high anxiety and either a long-term condition or high primary care utilisation, supporting residents to resolve financial stressors, improve housing stability and strengthen their capacity to manage their health.\n• Central West Croydon - Childhood Immunisation Equity Programme\nDelivering culturally competent outreach and navigation support for families from Black and minority ethnic communities with children aged 0-5 who are not up to date with MMR vaccination. The programme is designed to address key health inequalities identified through locality population health analysis and will focus on interventions that improve access to healthcare and address wider determinants of health.\nThe proposed service consists of three targeted prevention programmes aligned to the differing population needs of Central East and Central West Croydon:\n• Central East Croydon - Digital Inclusion and NHS App Uptake Programme\nSupporting residents aged 40 years and over who experience digital barriers to accessing health services and who are not currently registered with the NHS App. The programme will provide targeted outreach and personalised support to enable residents to access NHS digital services, improve self-management of long-term conditions and strengthen access to healthcare.\n• Central West Croydon - Welfare, Financial and Housing Advice Programme\nProviding structured casework support for adults living in IMD 1-3 areas who experience financial or housing insecurity and whose wellbeing is significantly affected by these pressures. The programme will prioritise individuals with moderate-to-high anxiety and either a long-term condition or high primary care utilisation, supporting residents to resolve financial stressors, improve housing stability and strengthen their capacity to manage their health.\n• Central West Croydon - Childhood Immunisation Equity Programme\nDelivering culturally competent outreach and navigation support for families from Black and minority ethnic communities with children aged 0-5 who are not up to date with MMR vaccination. The programme will work with communities and GP practices to improve uptake of routine childhood immunisations and reduce inequities in protection from vaccine-preventable disease.\nTogether these interventions aim to improve population health outcomes by strengthening access to NHS services, addressing wider determinants of health, and reducing inequalities in preventative healthcare uptake.\nThe service will operate through targeted outreach, structured behavioural and casework interventions, and coordinated referral pathways with primary care, voluntary sector organisations and local authority services to ensure residents receive timely and appropriate support.\nThe total indicative financial envelope for this service is £209,063 per annum.\nFurther detail regarding the proposed cohorts, delivery model and outcomes framework is set out in the Draft Service Specification provided in Appendix A.\nRESPONSE INSTRUCTIONS \nOrganisations should ensure that they have read the supporting information provided prior to responding, noting that the information and any intentions shared may be subject to change (in both form and content) should the ICB decide to procure a new service.\nPlease provide informative, yet succinct responses.
