NHS Nottingham and Nottinghamshire Clinical Commissioning Group is inviting providers of Domiciliary Care Services for Adults eligible for NHS Continuing Healthcare including Fast Track to a virtual Market Engagement Event on Wednesday 25th August from 13:00-14:30.
The service will be provided for people (Service Users) who have been determined by the Commissioner as having an NHS-funded continuing care entitlement.
The objective of the service is personalised care that is safe and promotes a good quality of life, meets assessed needs and contributes to the outcomes identified for each individual Service User. Also, to contribute to the reduction of inappropriate hospital admissions where patients have expressed a wish to be cared for within their own home.
