Providers will deliver a quality and effective Service personalised to each C/YP’s Assessed Need/s. The support from the Provider must contribute to enabling the family to live and feel supported in their family life with individualised Care/Support that reflects the C/YP’s needs. Providers will produce their own Care Plan and individual Risk Assessments personalised to the C/YP. The partnership between the family and the Provider will aim to maximise the potential of each C/YP to assist with daily routines and support the parent/carer to be able to successfully maintain their primary caring role. The focus of the Service is to promote independence, social inclusion, safe care, positive risk taking, and general wellbeing of the C/YP and their family.
