Our Community Dementia Support Service is designed to embed stronger integration across neighbourhood and frailty models of care, ensuring people living with dementia receive coordinated, person-centred support close to home. Working collaboratively with primary care networks, community health teams, social care, voluntary organisations, and local partners, the service promotes proactive identification, shared care planning, and seamless communication across disciplines. By aligning with frailty pathways and neighbourhood-based multidisciplinary teams, we enable earlier intervention, reduce avoidable crises, and support individuals and their carers to maintain independence, wellbeing, and quality of life within their own communities. In response to the unique characteristics of Sandwell the service is to be provided through a "Provider Collaborative Model". A Lead Provider is required to have sub-contracting arrangements in place with other community groups/ organisations across Sandwell who collectively are representative of the local communities' and diverse population mix that are to be served. The Service will provide a single point of access to support the health, wellbeing, and independence of those accessing the Service in community settings, including care homes and hospital settings. The service will provide information, advice, and referrals to other services to promote independence. The service will develop integrated care and support plans to enable people to be self-managing. The Service will deliver training for people living with the effects of dementia including carers and those with a memory concern to enable them to become self-managing. Training will be provided to professionals to address any identified Service deficits and ensure services are being provided by appropriately trained and skilled people. Training will also be provided for local communities and businesses in support of delivering "Dementia Friendly Sandwell" and schools and colleges to support destigmatisation.

"description": "Sandwell Metropolitan Borough Council (SMBC) and the Black Country Integrated Care Board (BCICB) are committed to commissioning high-quality, cost-effective services that meet the diverse needs of Sandwell's residents. These services aim to make optimal use of market capabilities and strengths, while ensuring full compliance with statutory and legal obligations.\nSMBC is currently exploring options for the future procurement of the Sandwell Community Dementia Service. The primary objective of this Service is to support early diagnosis and deliver personalised interventions that reflect the unique needs of individuals and the diversity of Sandwell's communities. These interventions will empower individuals to make informed decisions about their care and support, enhancing their overall quality of life. The Service will also focus on preventing crises, and in the event of a crisis, will collaborate with health and social care professionals to manage and resolve the situation effectively.\nThe Service will be available to individuals experiencing memory concerns-including those with mild cognitive impairment (MCI)-and those living with dementia, as well as their carers. Staff will work to enable and empower service users to pursue lives of meaning, value, and purpose. Delivery will be holistic and integrated, considering the whole person and their community, rather than focusing solely on symptoms or diagnosis. The Service will also support individuals in becoming more self-managing where possible. Support will be provided throughout the entire dementia journey, including direct engagement with carers-even in cases where the individual with dementia does not wish to receive support or is no longer able to participate. "description": "Sandwell Metropolitan Borough Council (SMBC) and the Black Country Integrated Care Board (BCICB) are committed to commissioning high-quality, cost-effective services that meet the diverse needs of Sandwell’s residents. These services aim to make optimal use of market capabilities and strengths, while ensuring full compliance with statutory and legal obligations.\nSMBC is currently exploring options for the future procurement of the Sandwell Community Dementia Service. The primary objective of this Service is to support early diagnosis and deliver personalised interventions that reflect the unique needs of individuals and the diversity of Sandwell’s communities. These interventions will empower individuals to make informed decisions about their care and support, enhancing their overall quality of life. The Service will also focus on preventing crises, and in the event of a crisis, will collaborate with health and social care professionals to manage and resolve the situation effectively.\nThe Service will be available to individuals experiencing memory concerns—including those with mild cognitive impairment (MCI)—and those living with dementia, as well as their carers. Staff will work to enable and empower service users to pursue lives of meaning, value, and purpose. Delivery will be holistic and integrated, considering the whole person and their community, rather than focusing solely on symptoms or diagnosis. The Service will also support individuals in becoming more self-managing where possible. Support will be provided throughout the entire dementia journey, including direct engagement with carers—even in cases where the individual with dementia does not wish to receive support or is no longer able to participate. Eligibility for the Service will include residents of Sandwell who are registered with a Sandwell GP practice, as well as their carers, regardless of whether the carers reside within Sandwell. \nThis preliminary market engagement seeks to understand supplier capabilities, gather feedback, and inform the development of a potential future procurement strategy.\nThis will take place on Monday 10 November 2025 from 11.00-12.00 for all interested organisations.\nPlease email by 15.00 on Friday 7 November 2025 rupi_deo@sandwell.gov.uk your full name, job title, organisation, email address and telephone no. if you would like to receive an online MS Teams invitation to this event.\nThank you for your interest.",
