The platform will provide the interface between patients and clinicians with the ability to enable remote monitoring.\nSurrey Heartlands ICB are seeking to procure a digital platform to cover an initial 200 Virtual Ward Beds and in addition, to reserve the right to commission further services from the successful provider covering:\n•\twearable remote monitoring devices and associated connectivity to the platform\n•\tincorporation of existing patients\n•\tadditional virtual ward beds\nIt is a requirement that the Digital Platform is implemented and operational in each of the four Places within 12 weeks of contract start date of 1 September 2023 and this is estimated to be by 31 December 2023. The digital platform is therefore estimated to be implemented and fully operational by 31 December 2023.\nThe Contract term will be 24 months initially, with the option to extend by any further period or periods up to a maximum of 36 months.
