The provider must have excellent relationships with other key services for vulnerable individuals such as housing, drug and alcohol, mental health and street support interventions and will play a crucial role in the development of integrated services for this population group by collaborating with other NHS providers and non-NHS services. This service also forms parts of Leeds CCG strategy to tackle health inequalities. During the contract term the provider will work with the commissioners and other stakeholders (including NHS and non-NHS providers) to create a broader service offer to the most vulnerable, focusing on wider health and wellbeing needs. This may include the establishment of a new healthcare facility where sustainable services can be delivered from.
