The aim is to enhance health, promote long-term skills, build supportive communities and empower self-management.<br/><br/>The award is being made to the existing provider of the service, as per Provider Selection Regime, Direct Award Process C, for a period of 18 months at a value of £107,663.25 to Living Well.
