What is being bought
North Northamptonshire Council has agreed £28,000 extra funding to enable providers to deliver 560 targeted health checks (THC) in community settings over three months, with the flexibility of a further extension. The programme will gather evidence on the feasibility and impact of conducting THCs.
The THC programme will provide an intervention to support and offer advice to target populations to help prevent the onset of CVD and help identify their risk of contracting CHD, stroke, kidney disease and diabetes. The intervention will be given to eligible people who are not currently on a CVD register or being treated as at-risk and are aged between 40 and 74. The objective is to support individuals from target populations in managing and reducing behavioural risks and associated conditions through information and an evidence-based risk reduction intervention.
