What is being bought
The Borough Council of Calderdale wishes to commission the mandated NHS Health Checks service for Calderdale to be delivered by a single or lead provider. The service will be based around delivery in general practice plus some community delivery outside of general practice with resources targeted to people who are likely to benefit the most. The aim of the service is to improve the health and wellbeing of adults aged 40-74 through the assessment and management of the major risk factors for cardiovascular disease. The provider will proactively seek to address inequalities in access to and outcomes from the NHS Health Check by working with other local partner organisations. The service will prioritise supporting people to understand and manage their risk rather than focus on case finding people with previously undetected conditions. The provider will be accountable for performance against the three national service KPIs relating to invitation rates, take up and coverage.

    The Borough Council of Calderdale is commissioning the nationally mandated NHS Health Checks Service. An award has been made to the existing provider following Direct Award process A. The core service offer will be delivered by a single or lead provider across all 20 GP practices in Calderdale with some additional delivery in other community settings. Clinical eligibility of service users must be ensured by way of a search of primary care clinical records. The provider will work with local partners to ensure the service is accessible by and effective for people who stand to benefit the most from the NHS Health Check. The lifetime value of the contract is up to £526,000 (including the optional 12 month extension period) dependant on activity levels. The contract provides for the service to be provided for two years from 1st April 2024 to 31st March 2026 with the option to extend for a further year until 31st March 2027.
