It will deliver to a health and care professionals, at the point of care, all of the information that is relevant to the care or treatment needs of that patient/citizen (people). Record access will be quick and easy, from the professional’s own system – no need to sign on to a different system. This means being supported by a team who have access to all relevant health and care information quickly, easily and in as near to real time so they can proactively keep people well both physically and mentally and can intervene early where there are risks of crisis, escalation or other clinical risk. We want every service user to have confidence that when they tell their story all of their care providers will have access to it so they don’t need to say the same thing over and over again. Currently, data integration is the biggest challenge for system partners due to numerous EPR and clinical documentation systems.
