The service should include provision of easily accessible, ongoing specialist advice information and support, alongside clinical interventions that enable people with dementia and their family carers to experience improved health and social care outcomes and an enhanced quality of life. The service must be able to demonstrate understanding, knowledge, and expertise in relation to dementia, and its impact on families affected by the condition. It must also demonstrate an ability to use a person-centred approach delivering the most appropriate level of support at the right time. The service model should include a clinical component provided by skilled specialist practitioners who provide biopsychosocial support to families with complex needs alongside specialist education and advice to health and social care colleagues.
