What is being bought
"The Health and Rehabilitation Programme (HARP) is a model of rehabilitation care which has been designed to support individuals with multi-morbidity to be engaged and supported to manage their health and wellbeing through access to a rehabilitation programme. The project involves a multi-professional, multi-agency approach.
Each participant referred receives 3 consultations per year. An initial consultation upon referral where the following data is collected: height, weight, Body Mass Index, waist circumference, blood pressure, resting heart rate activity levels (SPAQ questionnaire), health and well being (EQ5D questionnaire). At follow up health and wellbeing questionnaires are repeated. Participants who are referred to the programme for falls prevention undergo additional assessments to assess their balance, strength and co-ordination. Assessment tools used include the Tinetti assessment tool which is widely recognised by physiotherapy.
