Hospital @ Home is a future cornerstone of inter-linked and place-based teams who support patients and colleagues in the community across Somerset. The Hospital @ Home team, whilst having discrete components that will develop in a phased way over time, will be collaborative and work as a whole team wrapping care around a person and those important to them. The core components will provide safe support for patients with, but not limited to, frailty, respiratory diseases, cardiovascular disease (including heart failure) and complications of diabetes.
