It aims to build health literacy and encourage self-care strategies. Targeted Support provides structured one-to-one and group programmes for individuals needing more intensive help, including rehabilitation for long-term conditions and behavioural interventions linked to NHS Health Checks. Specialist Services focus on evidence-based interventions for weight management and smoking cessation. Weight management support will follow NICE guidance, offering structured behavioural programmes for adults with a BMI of 25-39.9 (or 23+ for ethnic minority groups). Stop smoking services will deliver behavioural support, pharmacotherapy, and harm reduction strategies, including NRT and vapes, in line with NICE guidance. Key deliverables include a high-profile universal programme, targeted interventions, a specialist weight management service, and a community stop smoking service with multiple referral routes. The provider must ensure accessibility, maintain strong partnerships with local health, care, and community networks, and operate flexibly to meet evolving needs.
