What is being bought
This application was for the 2nd opening of applications for LOT 2 – Transforming Care Project and is for providers of support to people with a learning disability and/or autism who display behaviour that challenges, including those with a mental health condition. Norfolk County Council has established a multi supplier framework agreement for the provision of supported living accommodation and support.Transforming care is all about improving health and care services so that more people can live in the community, with the right support, and close to home.Successful Providers for Lot 2 will provide supported living care and support packages for individuals leaving hospital or at risk of admission to secure in-patient facilities arrangements in the future
