This notice is a Prior Information Notice under the permitted procedure for social and other specific services, as provided for in the Public Contracts Regulations 2015 - Regulation 48. In accordance with Regulation 32(b)(ii) of the Regulations, interested parties should note that the contract for the services may be awarded without further publication and interested parties should express their interest in writing by following the instructions in this document and responding to all questions accordingly.<br/>Providers will be required to respond to a series of questions which are scored on a "Pass/Fail “basis. The scoring criteria is stipulated within the question.<br/>Only providers who score a "Pass" on all "Pass/Fail" questions will be deemed compliant and capable.<br/>Where a single Provider has expressed an interest and met the requirements, the Commissioner reserves the right to direct award to that provider with no further competition.<br/>Where there is more than one Provider who has expressed an interest and scored a "Pass" on all "Pass/Fail" questions, then the Commissioner may seek to undertake a formal competitive process.<br/>The Authority will be using an eTendering system for this procurement exercise. You can search for the opportunity by entering the following contract reference: C163913 ID number.<br/><br/>Service Description <br/><br/>The objective is to identify AP interventions at the earliest opportunity and to work with the Provider to provide home and/or community based AP Interventions; identification is clinically determined individually in consultation with Service Users and their Families or Carers. <br/><br/>This service aims to: prevent admission; reduce the length of stay; facilitate wrap around care / step-down care following discharge; by providing community-based AP Interventions to Service Users identified as “highly likely” to need admission, in collaboration with external organisations. It can also be used to bridge the gap whilst Service Users are waiting for another service to become available – for example social care interventions. It is agreed that AP Interventions will be brief, and time limited to six weeks of support.<br/><br/>The Services to be provided by the Provider are delivery of AP Interventions which meet the needs of Service Users to prevent an admission to an acute mental health hospital or reduce the length of stay following an admission to an acute mental health hospital (the “Services”) or to support discharge back into the community. <br/><br/>The Provider shall work in partnership with three Trust service lines: <br/>• Norwich and North Norfolk Care group<br/>• West and South Norfolk Care group<br/>• Great Yarmouth and Waveney Care group<br/>• Norfolk CFYP Care group (for 18+ service users only)Contract periodNot publishedEligibilityNot publishedOptions / renewalNot published
