This service is Primary Care led, in collaboration with local system partners, and is to deliver an integrated Enhanced Health service in all care homes (residential and nursing), as registered by the Care Quality Commission (CQC), supported by East and North Practices, and which are covered under the PCN DES/DES plus. Inclusive of non-weight bearing and Discharge to Assess beds (DTA) as well as Learning disability adult care homes. Exclusive of short stay respite services, neurological services, and flexi care schemes.<br/><br/>The contract shall commence on 01 April 2025 and end on 31 March 2028.<br/><br/>This contract is a 2 year pilot (Contract value £1,644,000) with an option to extend for a further 1 year (Contract value: £822,000) at the discretion of the commissioner.
