Provision of a Community Stroke Rehabilitation and Early Supported Discharge (ESD) service for patients recovering from stroke. The service provides multidisciplinary rehabilitation following hospital discharge, supporting patients in their own homes and community settings to maximise independence, improve recovery outcomes and facilitate timely discharge from acute stroke services. The service operates in alignment with the Integrated Community Stroke Service (ICSS) model and includes delivery of rehabilitation through home-based, clinic-based, digital and group-based approaches.

- Change This contract modification introduces additional recurrent funding of £550,782 per annum to support expansion of the Community Stroke Rehabilitation workforce and implementation of the Integrated Community Stroke Service model. The modification funds an additional 7.5 WTE multidisciplinary workforce comprising nursing, therapy, rehabilitation assistant and administrative roles. The modification also updates the service specification, introduces Service Development Improvement Plan (SDIP) and Data Quality Improvement Plan (DQIP) requirements, and establishes new performance and outcome measures relating to responsiveness, therapy intensity, telerehabilitation and group rehabilitation delivery. There is no extension to the contract term. The modification takes effect from 1 September 2026. — Need for additional works, services or supplies by the original contractor/concessionaire: The current service is provided by ECCH. This is an enhancement to the existing service
