The Service will aim to improve the quality of life for carers through an outcomes led approach, providing the right level of intervention, at the right time. The Service will aim to improve the quality of life for carers through an outcomes led approach, providing the right level of intervention, at the right time. The Service will be flexible and designed to meet the needs of carers in Aberdeenshire. Key Service Aims: - Identification of carers through contact with the Health and Social Care teams, Education and Children’s Services, other third sector organisations and development of innovative ways to engage with hidden or marginalised carers; - Provide carers with information, advice and enable them to access resources within their communities; - Support carers to complete either an Adult Carer Support Plan or Young Carer Statement; and where this is declined, record and plan other support that is available through the service - Support carers to complete either an Adult Carer Support Plan or Young Carer Statement; and where eligible refer to the carers team for an SDS budget. Where carers are not eligible for SDS the service will provide support to enable carers to meet their identified outcomes - Support carers during transitions within their caring role; - Support at the point of hospital discharge; - Manage short break funding provided by the Scottish Government through Shared Care Scotland; - Carer involvement and consultation; - Support carers to maintain their own health and well-being; - Provide training and development opportunities to enable carers to continue in their caring role; - Development role.Contract periodNot publishedEligibilityNot publishedOptions / renewalNot published
