These beds will meet the health and care needs of patients over the age of 18 who are currently in hospital but no longer require acute hospital care, but who cannot be immediately discharged to their own homes without a further period of bedded recovery, rehabilitation and assessment. Nursing, personal care and therapy input will be provided in a flexible, multi-disciplinary way which adapts to meet a range of requirements depending on the assessed needs of each patient. Some patients will require more intensive rehabilitation and some will require a period of recuperation and recovery with a re-abling approach. The aim of the service is to maximize the potential for each patient and to work with partners to facilitate the timely transfer of patients back home or into a long-term placement at the earliest appropriate opportunity. We will expect the provider to actively plan with the Multidisciplinary team to ensure goals are clearly set and length of stay is minimised.
