## Description
This system is used by referrers to request if an individual is eligible for Continuing Healthcare, the individuals care record is stored and edited whilst they are eligible, care is sourced and added to the system showing provider and rates. It allows BI to pull reports to understand how the AACC is meeting the NHS Framework standards such as 28 days. It is the only system used by AACC to capture all the necessary information about an individual
