The Patient Transport Hub promotes access to healthcare provision through an NHS funded transport service for those patients registered with a GP in NHS Bristol North Somerset and/or South Gloucestershire who meet the eligibility criteria as set down by NHS England and the Department of Health and Social Care (DHSC). Eligibility criteria includes vulnerable people by reason of mental health or learning disability, children, and eligible escorts. The role of the PTH Provider is to liaise with patients/carers and subject to eligibility criteria book transport for qualifying patients and or signpost them to alternative modes of transport. Full details including specification, activity, scope and requirements can be found within the procurement documentation. The Commissioner has a budget of £400,000 per annum. The total contract value for the 5-year Contract is £2,000,000. At the end of the initial contract term the Commissioners will have an extension option for any number of periods up to a further 2 years available which will be awarded at their discretion, giving a maximum possible contract value of £2,800,000 over 7 years if the full contract term is fulfilled.
