## Description
An Arts & Health programme (visual arts and crafts/mixed media programme) will be offered to patients who regularly present at GP
surgeries in Ceredigion, Pembrokeshire and Carmarthenshire living with a chronic condition and/or pain, social isolation and mental health
difficulties as a result of circumstances/condition to improve health and wellbeing.
