The service will seek to address key modifiable risk factors that contribute to preventable ill-health. The service will have a primary focus on physical activity, diet and excess weight, strength and balance for falls prevention and earlier identification of increased risk of developing long-term conditions. The service will introduce a central triage function to improve access and referrals into healthy lifestyle support, along with an expanded range of interventions to support behaviour change.
