Specific requirements of the service/provider: • To deliver high quality, patient centred primary medical services, via an Alternative Provider of Medical Services (APMS) contract to the *'indicative' patient population of Townfield Health Centre located in Birkenhead, Wirral. • Delivery of essential, enhanced primary medical services (as per the draft specification) and local incentive schemes. This includes the provision of the Special Allocation Scheme (previously violent patient scheme). • Delivery of safe and effective care within a learning environment. • Work collaboratively with system partners, Primary Care Network and in partnership with NHS Cheshire and Merseyside Integrated Commissioning Board (ICB) • Must be CQC registered. * it is worth noting that there is no formal registered list size for the practice currently due to a merged clinical system being in operation. Therefore, the indicative registered list size is 4,100 as at May 2024. Specific contract details; • The proposed contract term will be 10 years, consisting of break clauses at year 4 with two periods of 3 year extensions (4+3+3) over the duration of the contract. This provides protection for both parties in regards to satisfactory and viable contract delivery. • Anticipated contract start date - 1 April 2025 • Indicative list size of 4,100 (please see point note above marked *). • Practice location - Townfield Health Centre, Townfield Close, Birkenhead, Wirral, CH43 9JW.
