We would therefore like to invite engagement around the concept and service design at this early stage from any interested parties. As a provider, you will have a proven track record of working successfully with different client groups, hold the relevant CQC registration (where required) and have an existing operating presence within Wandsworth or the surrounding boroughs. Each client group will have varying support needs, so it is essential that the provider is able to deliver tailored support to: . Adults with a Learning Disability who may require physical and personal care and support to maximise their potential, build and maintain their life skills, personal development and quality of life. Adults with enduring Mental health needs who may require care and support to promote and maintain their recovery from mental ill health, promote their life skills, promote their independence and participation and integration in their local communities. Care leavers who may require tailored support to live independently and fulfil their potential in adulthood. Residents at risk of homelessness who may require short term support to address any factors that have led to a risk of homelessness and ensure they have the necessary life skills to reduce any homelessness in the future.
