SMD is defined as concurrent experiences of three or more of: homelessness, substance use, mental health issues, domestic and sexual violence or abuse, and contact with the criminal justice system. Nottingham City Place Based Partnership is committed to improving the outcomes for people experiencing SMD and aims to procure ‘Changing Futures’ services as part of meeting these needs. Nottingham has secured ongoing funding through the Integrated Care Board’s Health Inequalities Innovation Improvement Fund. The Services will be commissioned in two Lots: Lot 1 is the Main Delivery Service (MDS) Lot 2 is the Partnership Learning Hub (PLH). Providers are invited to tender for Lot 1 and/or Lot 2. Lot 1 - The Main Delivery Service comprises the required Core functions: Front Line Delivery a. Intensive Case Management b. Multi-Disciplinary Team c. Peer Mentoring System Development d. Lived Experience Coordination e. Learning and Evaluation Lot 2 - The Partnership Learning Hub: training and shared learning to all system partners and services in Nottingham.
