What is being bought
Nottingham City Council has redesigned the pathway of services for citizens who require support as a result of mental ill health. The new pathway will ensure a consistent offer of services for citizens and ensure that they have access to the services which will best help to support them to live healthy lives in the community. Services within the pathway will be strength based with a clear focus on supporting citizens to live independently. As a result of the above, Nottingham City Council is seeking to appoint a suitably experienced and knowledgeable service provider for the provision a service which is a time limited and highly focused intervention which involves one to one community support to help citizens to overcome issues in their daily lives impacting negatively on their mental health and wellbeing. Support will be provided for up to 12 weeks. The service will support service users to identify barriers / challenges to living independently and by utilising a strengths based approach, develop a support plan to enable citizens to remain independent in the community,or to facilitate a timely discharge from psychiatric wards. Service users may require support in the following areas; • welfare benefits and housing issues; • immigration status; difficulties • travel and transport or getting out of the house; • help to link citizens in to activities outside of mental health services • help with the transition from an acute hospital stay to living independently at home • activities of daily living within the home to enable citizens to maintain their environment • medication management including prompting Please note that this is not an exhaustive list and the service is expected to respond directly to the needs of the service users
