## Description
Shropshire Council, in partnership with the ICB, is seeking to commission a new preventative services contract aimed at supporting Shropshire residents aged 18 and over. This contract will adopt a preventative approach, delivering a variety of place-based, community-led initiatives designed to promote healthy lifestyles, address inequalities, and provide early interventions that enhance residents’ health and well-being. In doing so, it aims to reduce reliance on statutory services. The Local Authority intends to structure the contract around three themed lots, with an emphasis on strong collaborative working.
This tender will be split into 3 lots as follows:
Lot 1 - Supported Hospital Discharge 
Lot 2 - Community-Led Mental Health Outreach
Lot 3 - Wider Community-Led Wellbeing Offer
