{"schemaVersion":"1.0","id":"ocds-h6vhtk-051343","title":"Care and Support at Home Framework","description":"Bournemouth, Christchurch and Poole Council (BCP Council) is seeking providers to join the new Outcomes-Focused Framework for Care and Support at Home, designed to transform the way care is delivered across our communities. \nThis updated approach places people, their strengths, and aspirations at the heart of everything we do. \nBCP Council Adult Social Care's (ASC) vision is to support people to live fulfilling lives, in ways they choose, and in environments where they feel safe and connected.\nThe Care and Support at Home Framework is committed to a progressive approach that will embed outcomes focus throughout the duration of this contract.
